A Group/Team assessment allows multiple respondents to assess a shared subject (a team, a department, or a concept), with results aggregated into a single cohort report. Setting up a Group/Team assessment involves enabling cohort functionality, configuring the assessment as a group cohort, defining the participant roles, building the assessment content, and launching a Cohort Initiation.
The high-level setup process is:
- Enable Cohort functionality. In Site Settings > Features, confirm that Cohorts are enabled for your site.

Configure the assessment as a Group/Team cohort. Open the assessment and navigate to Assessment Settings > Cohort. Three sub-tabs appear under Cohort settings:
- Cohort Settings: Select Group from the Cohort Type dropdown. This is essential, as it determines how the rest of the cohort behaves. Also set Default Days to Completion and any minimum or maximum response limits as required.
- Cohort Roles: Add at least one role. Roles differentiate participants and can be used to filter the report, show different parts of the assessment to different roles, and send tailored emails. Common roles include one role per job function in the team, or a single role named "Respondent".
- Cohort Iteration: Optional. Configure this only if the group will be reassessed over time to track change.
- Configure the assessment questions. Build the questions, sections, and subsections in the Assessment Builder as you would for any assessment. For full details, see the article links at the bottom of this page.

Set up cohort email templates. In the assessment's Emails tab, create the email templates required for the group cohort:
- For respondents: Welcome, Auto Reminder (optional), and Completion (if you are also sending individual feedback reports).
- For the cohort contact: Cohort Feedback and Cohort Progress Report.
- Configure the cohort report template. Build or assign a Word report template designed for cohort reporting. Confirm the report uses cohort-appropriate merge strings.
- Test and launch a cohort. Go to Cohorts, click Add Cohort, and add a small number of test respondents to verify that invitations, reminders, completion emails, and the cohort report all work as expected. Once the test cohort is verified, launch a Cohort Initiation for each real group by specifying the subject, the respondents in each role, and the completion date. The cohort report is produced on the day after the completion date, provided the minimum response criteria has been met. It can also be run manually from the Cohort Report Manager.
