The most common cause is that the question used to collect the respondent's name (or email address) has not been linked to the corresponding field in Response Settings. When a question is not linked, the value the respondent enters is captured in the response itself, but it is not populated into the dedicated Name or Email fields used by reports, response lists, and email templates.
To resolve this:
- Open the assessment in the Builder.
- Locate the question used to collect First Name, Last Name, or Email Address.
- Click the question to open its settings, then open the Options tab.
- Under Options, locate the Response Settings link or selector and link the question to the appropriate response field (First Name, Last Name, Email Address, etc.).
- Save the question.
- Test the assessment by submitting a new response and confirm the value now appears in the correct column of the response list.
Existing responses submitted before the link was configured will continue to show blank values. Only new responses, submitted after the link is in place, will populate the fields correctly.
