Use this checklist to verify your individual assessment is fully configured and ready to go live. It covers response settings, assessment structure, scoring, ratings, emails, and feedback reports.
Note: This checklist covers the essentials for a standard individual assessment. If you are using cohorts, 360s, group/team assessments, or iterations, additional configuration is required. 1. Response Settings (Welcome Page)
If you collect respondents' information in section 1, check the questions are linked correctly to Response Settings:
- Name field is configured (visible / required as needed)
- Email field is configured (visible / required as needed)

2. Assessment Structure
- At least one Section exists (each section = one page for the respondent)
- Each Section has at least one Subsection
- Questions are added within each Subsection
- Question order and flow make sense (preview the assessment)

3. Scoring and Answers
- Each scored question has answers with correct point values assigned
- Any non-scored questions (open text, instructions) are intentionally unscored
- Verify total possible scores make sense for your rating scale
4. Ratings
- Rating scale is configured with appropriate ranges and labels
- Rating text is written for each rating level (if using rating text in reports)
- Section or Subsection ratings are configured if reporting at that level
5. Email Templates
- Welcome Email is set up (Email Type: Welcome)
- Completion Email is set up (Email Type: Completion)
- Email content is customized (subject line, body text, merge strings for links)

6. Feedback Report
- Report template (Word document) is uploaded on the Feedback tab
- Merge strings are inserted correctly using the Merge Manager Add-in
- Report generates without blank or missing sections
Tip: If you are using a Results Dashboard instead of (or in addition to) a feedback report, verify that the dashboard is configured and displays correctly. Read more about Results Dashboards

7. Test and Review
- Create a new test Response from the Response List screen
- Complete the assessment as a respondent would
- Verify the welcome page fields work as expected
- Check that scores and ratings calculate correctly
- Generate the feedback report and review it end to end
- Confirm the welcome and completion emails were sent (check Sent Emails)
