How do I add my Questions?
- Make sure you have a section and a subsection.
- Sections equate to pages on the response and are a level of aggregation in the scoring and rating. (Read more about creating sections)
- Subsections are a group of questions on the same subject, and are important for scoring, rating and the building of feedback text. (Read more about creating subsections). You can have any number of subsections in a section - but you need at least one.
- If you hover your mouse on the subsection or you go to the bottom of the page (once a subsection has exists) you will see the Add Question button

Should I have a Welcome Page?
Yes, we recommend setting up Section 1 as a Welcome Page where you collect the respondent's First Name, Last Name, and Email Address.
It is important to collect these details on the first page (Section 1) because if you share your assessment externally - for example, via an external link on your website - the system needs the email address on the first page to trigger the Welcome Email template. If the email field is on a later section, the Welcome Email will not be triggered for externally shared assessments.
Tip: Once you have added your First Name, Last Name, and Email questions, make sure you link each question to the correct Response Setting field. This ensures the data is stored properly against the response record. Read more about Linking Questions to Response Settings.
How do I add my Feedback Report Template?
The Feedback report is added in the Settings, which is available via the 'Settings' button in the assessment builder:
