We have a collection of sample report templates that you are welcome to use for Feedback or Cohort Reports. Please be aware that if you use the sample report templates, you will still need to update the merge strings to work with your assessment structure.
Some of the sample templates have images on them that work with our demonstration assessments. Normally you would substitute them with Images suitable for your assessment. We have left the existing pictures in the template as it is easier to change them (using the Change Picture menu item when you right-click the picture) and the new picture will retain their attributes (often behind text etc.).
The Videos show each below show each template populated with demonstration data. Once you have chosen the template you can download it at the bottom of this document.

Template 1 is a very simple Individual Feedback Report Template - low on graphics.

Template 2
Template 2 is an Individual Feedback Report Template and includes the option to show comparative results using benchmarks alongside the respondents' scores.

Template 3 is an Individual Feedback Report Template and includes the option to show comparative results using benchmarks alongside the respondents' scores.

Template 4
Template 4 is an Individual Feedback Report Template and includes the option to show comparative results using benchmarks alongside the respondents' scores.
Template 5
Template 5 is an Individual Feedback Report Template.
Template 6
Template 6 is an Individual Feedback Report Template and includes the option to show comparative results using benchmarks alongside the respondents' scores at assessment and section level. The template also includes the option to show a table detailing the respondents' answers to specified questions with recommendations based on their answers.
Template 7
Template 7 includes a spider chart and the option to show benchmarking and a question level breakdown. Add in some rating text and recommendations for a solid all round report option.
Template 8
Individual Feedback Report Template includes top performing versus growth areas and results for the assessment, sections and subsections.
Template 9 First Iteration
Template for reporting on the first iteration results for a group cohort.
Template 9 Last Iteration
Template for reporting on the first and second iteration results for a group cohort.
Template 10
Template 10 is set up as a single respondent cohort to show iteration results, comparing the respondents' results over time.
